Yes. See CBCA, S. 191, link below.
2. If yes to #1 does it make it more difficult to get the 25% votes needed to stop the "merger" voting?
Yes, by definition, as someone who wanted the offered securities via merger would not dissent.
3. Once you obtain certificate how do you file a dissent? Is there a form? And how can i obtain such form if there is one?
The procedures are spelled out in the CBCA, and will also be fully described in the Management's Information Circular.
4. What is the mailing address of such authorities were certificate and dissent notice must go?
That will be in the MIC.
5. Lastly can shareholders vote against a dissent in the supreme court if the fair value proposed by mediator does not seem fair to shareholders? In such case what would happen?
The fair value assessment, if it goes to court, will be argued in the Superior Court of Justice. You must be represented by a lawyer, so it can get expensive. Any decision of the Superior Court can be appealled to a higher court, but there is no guarantee that the appeal will be heard.
